Hair loss gets sold harder than almost anything else in this industry, usually before anyone has established what kind of hair loss it is. There are several, they look similar at arm’s length, and they want opposite treatments.
The first visit
Trichoscopy across four scalp zones — we are looking at follicular ostia, hair shaft diameter variance, and whether the openings are still there at all, because that single question separates the treatable from the permanent.
A pull test, a scalp examination for inflammation and scale, and a blood panel: ferritin, full blood count, thyroid function, vitamin D. Ferritin under 30 will cause shedding on its own and is found often enough that we test for it every time.
What we prescribe, and when
Where androgenetic alopecia is established, topical or oral minoxidil and — where appropriate and after a proper conversation about side effects — a 5-alpha reductase inhibitor. Where there is inflammation, that is treated first; growing hair out of an inflamed scalp does not work.
You get photographs at the same four zones, and at three months we compare rather than ask how you feel it is going.
The honest limit
Where the follicular openings have gone, the follicle has gone, and nothing we prescribe will bring it back. We will show you the trichoscopy image and say so. What medication can do is protect what is still there, which is worth a great deal — but only if you were told the truth about which is which.
